Fixed Bug wrong URL was used to check host availability

When authentication is turned on the URL used to check for server
availability was incorrect. We used the truncated version of the url
without credentials. We need to use the URL with the valid credentials
in order to register hosts with auth turned on.

The docker-machine and go-machine-service binaries were added
to the server image. These are necessary to enable machine based
services in Rancher.

Mysql added to server

Handle configuring/starting/seeding mysql as the default option. In
this pass, we are handling Links, Runtime Variables and with in the
container. A future iteration will include service discovery.
